Reporting an injury
If you have sustained an injury on the job without seeing a physician or missing time from work, it is important that you report this injury to your supervisor and complete the Departmental Incident and Hazard Report form. This ensures that the incident has been recorded and is available in the future should you need to submit a WorkSafe claim. Minor injuries can be documented in the Department Accident Record Book.
If you have sustained an injury that requires medical attention from a physician, the physician will bill WorkSafe for the services provided, thus initiating a WorkSafe claim on your behalf. You will need to complete a Form 6A – Worker’s Report of Injury to Employer and provide this form to your department. This form is strictly for the University and will not be forwarded to WorkSafe. Your department must then complete a Form 7 – Employer’s Report of Injury. Both forms must be submitted to Occupational Health, Safety & Environment (OHSE) as soon as possible for processing. In addition to these forms, you will need to report your injury to WorkSafe directly at 1-888-WORKERS (1-888-967-5377). If you do not call them directly, they will contact you by telephone or mail to request a Form 6-Application for Compensation & Report of Injury. This additional form must be completed and sent directly to WorkSafe or your claim will be suspended. Once your claim has been processed by the University, WorkSafe will adjudicate the claim and send a decision letter directly to your home address.
In order to prevent a similar injury from occurring again, a mandatory incident investigation will be conducted. Your supervisor will be contacted to arrange a time for the investigation meeting.
If your injury is such that you may require an accommodation in order to facilitate your return to work, please have your department contact your Work Life Consultant in Human Resources.

Incident investigations
Incident investigations
In order to prevent workplace injuries, the University conducts an Incident Investigation. The purpose of the investigation is to identify any corrective measures that can be implemented to prevent the recurrence of the incident.
Joint Incident Investigations are arranged by Occupational Health, Safety and Environment (OHSE). The investigation involves you (the injured), an Employer Representative (your supervisor), a union/employee group representative, and an OHSE Consultant.
The Departmental Incident and Hazard Report form will be completed during this meeting and provided to the OHSE Consultant. This report will be available to WorkSafe for review as well as the appropriate joint local safety committee.
